Susan asks all new clients to fill out a "New Client Questionnaire" in order to gather relevant information that may be needed to create their Estate Plan. Please bring the completed form to your first meeting.

Please be aware that you have not hired Susan until you sign a retainer. Simply filling out the questionnaire does not establish an attorney-client relationship--it is for informational purposes only. Note: you need to have Adobe Reader installed on your computer in order to access this document.

Instructions for downloading the New Client Questionnaire:

Step 1: After clicking the "Click here to download the New Client Questionnaire" (above), the questionnaire will open in a new tab on your internet browser. This instruction page will stay open in a different tab. The new tab might look a little like the Photo 1 on the right. (You can click the photos to enlarge them.)

Option 1, step 1: If you would like to print the questionnaire so you can fill it in by hand, click the printer button (see Photo 1, blue circle). If you decide to do this, you do not need to follow the rest of these instructions. Bring the completed form to your meeting.

Option 2, step 1: If you would like to type the information, click the download button (see Photo 1, red circle). 

Option 2, step 2: Once you've downloaded the questionnaire, open it and save it (see Photo 2, purple circle) on your computer with a name and location that you will remember (see Photo 3). 

Option 2, step 3: Fill out your information by clicking rectangles and typing your answers. Some yes/no answers just require you to click a box to add a check mark. 

Option 2, step 4: Once you have completed the questionnaire, remember to save it again. This time you shouldn't be asked to rename your document, simply clicking the save button (see Photo 2, purple circle) should do it.

Option 2, step 5: Print the completed document and bring it to your meeting (see Photo 2, orange circle).
